Workgroups

Infant & Maternal Health: Did you know that Waterbury has one of the highest rates of infant mortality in the state of CT? The Infant & Maternal Health Workgroup is working to improve birth outcomes and maternal healthcare for all women, and decrease the infant mortality rate through outreach, education and access to prenatal and postnatal care.

Chronic Disease: The Chronic Disease Workgroup focuses on the outreach, education, prevention and management of chronic disease including asthma, obesity, diabetes and hypertension.

Access to Care: Through coordination with Waterbury Health Access Program and the CCT, GWHP addresses the medically underserved to reduce emergency room utilization and improve care for extremely challenged patients that are often homeless and suffer from co-occurring conditions.  

Substance Abuse: In cooperation with the City of Waterbury Prevention Coordinator, GWHP works with the community providers and CCT case manager partners to address and refer to treatment options, review community data and examine strategy effectiveness in the region.
