GWHP partners with other community based organizations and clinical partners through grants and shared funding to achieve the most collective impact in the region. GWHP’s involvement in collaborative projects fall into one of the three health priority areas: Health Risk Factors, Health Influencers and Access to Care. Some of our recent collaborations include:
