In 2019 GWHP continued upon early work around the State of CT’s State Innovation Model initiative Health Enhancement Communities (HEC). The Partnership worked with over 20 community partners over the course of two phases and 12 months to complete a feasibility study and report for implementation of this large-scale upstream health intervention- based approach. The State of Connecticut Office of Health Strategy has provided a 2-year funding through CDC funding streams for HECS to expand Health Equity work. GWHP is leveraging these dollars by expanding the Waterbury Health Access team to include additional CHW’s and Clinical Social Workers. This will allow more patients to receive access to care and connection to Primary Care.
